Public Consulting Group, Inc. ( PCG ) has been contracted by DCH to operate the Subrogation Unit for all of these programs. The Subrogation Unit is responsible for enforcing each program's rights to recovery in subrogation claims and refunds back to the State.
The Subrogation Unit's purpose is to recover medical expenditures from third parties or the member's legal representative in cases where members are involved in a personal injury matter such as auto accident, medical malpractice, general liability, worker's compensation or any other tort claim where a settlement has been awarded.
Please Print and complete the transmittal form, and fax it to our office at (770)937-0180. The Subrogation Unit will contact you with the subrogation amount to assist you in the settlement of your claim. If you require a breakdown of the bills paid by DCH, please include a signed medical authorization or you may print and complete the medical authorization found under the forms menu.
